MOC Training

The Centers for Medicare & Medicaid Services (CMS) requires that all providers who service patients enrolled in a Special Needs Plan (SNP) such as ProCare Advantage complete the Model of Care (MOC) yearly training. 

 

How are we doing?

We continuously seek improvement opportunities and would like to know how you feel about the services provided to you. By completing the Provider Survey, you will not only help provide insight to your experience with the plan, you will also be able to provide information on how you are meeting the plan standards for access & availability.
